A participating agency, a senior or their family member will create a Care Circle and invite other people to view the needs assessment and care plan activities, with one individual acting as the Care Circle Admin. If the senior manages their care needs, they will serve as the Admin. Otherwise, the appointed Admin will be responsible for helping develop and monitor the care plan over time.
